What actually happens
The tower stays up and Death files a complaint
The movie starts in 1969, when Iris Campbell and her boyfriend Paul go to the opening of the Sky View, a restaurant at the top of an observation tower. Iris has a premonition of the tower collapsing and killing everyone inside, warns the guests, and prevents it. That wrecks Death's plan. In the present day, their granddaughter Stefani Reyes, a college student, keeps having nightmares about the averted disaster and goes home for answers. She finds her father Marty and brother Charlie, then visits her uncle Howard, aunt Brenda and cousins Erik, Julia and Bobby. Howard tells her that he and her mother Darlene grew up sheltered and isolated by overprotective parents, which explains the family tension and why Darlene left. Brenda helps Stefani dig up letters from Iris, and they point to a fortified cabin where Iris is dying of cancer. Iris explains that after she saved the tower, Death began taking the survivors in the order they were supposed to die, and then their descendants. She kept a notebook of what she saw. Stefani doesn't believe it and tries to leave, so Iris follows her outside, hands over the notebook, and lets a weather vane impale her to prove it.

Howard meets his lawnmower
Darlene returns for Iris's funeral, and Stefani resents her for abandoning her and Charlie as kids. In the notebook Stefani finds a reference to someone called "JB", who knew someone that beat Death. At a family barbecue, a chain of events ends with Howard dead under his lawnmower in front of everyone. After his funeral Stefani tries to warn the family, but they don't want to believe her. A fire breaks out in Erik's tattoo shop, and he survives because of his leather jacket. Stefani and Charlie try to protect Erik, since they assume he is up next, but it is Julia who dies in a garbage truck's compactor. Brenda admits that Erik was born of an affair and has no connection to Iris's bloodline, which is why Death skipped him.
A malfunctioning MRI machine joins the family
Darlene suggests tracking down JB, and he turns out to be William Bludworth. As a child he was at the Sky View with his mother and would have been the last person to die in Iris's first vision. He tells the family there are two ways to beat Death: take another life, or be clinically dead and then resuscitated, as Kimberly Corman was. He leaves, expecting his own cancer to finish him once Iris's bloodline ends. Erik persuades Bobby to try the second method by triggering a deadly nut allergy and then reviving him. This is the scene people dread most, and it comes near the end of the movie. It goes wrong: an MRI machine malfunctions, rips the piercings out of Erik and pulls him in, a wheelchair impales and crushes him, and a coil from a vending machine shoots into Bobby's head. After that, Stefani, Charlie and Darlene head for Iris's cabin, hoping to hide Darlene there. They crash through the gate, Stefani's seat belt jams, the cabin explodes, and Darlene's RV goes into the water, where Stefani starts to drown. Darlene saves Charlie and is crushed by a lamp post. Charlie rescues and resuscitates Stefani.
The ending, explained
A week after the cabin, the father of Charlie's prom date, who is a physician, tells the siblings that Stefani never counted as clinically dead, since she was revived before her heart stopped. The method never applied to her. Moments later a freight train carrying timber derails nearby, and the cargo crushes Stefani and Charlie. By the end Iris, Howard, Julia, Erik, Bobby, Darlene, Stefani and Charlie are all dead.
