What actually happens
The bridge goes first
Sam Lawton is riding with his coworkers to a company retreat when, on the North Bay Bridge, he foresees high winds bringing the whole thing down and killing everyone except his ex, Molly Harper. Sam panics and talks Molly, Peter Friedkin (his friend and boss), Peter's girlfriend Candice Hooper, his manager Dennis Lapman, factory manager Nathan Sears and colleagues Olivia Castle and Isaac Palmer into leaving the bridge. The bridge then collapses. FBI agent Jim Block suspects Sam of causing it, until a colleague decides it was not man-made. At the aftermath, Sam and the others also meet a mortician named William Bludworth, who warns them that Death does not like to be cheated.
Candice dies in a freak accident during gymnastics practice, and Peter begins to fall apart. Isaac is next: a fire breaks out during a spa treatment, a shelf gives way and a Budai statue falls on his head. Bludworth is on hand after both deaths, and he explains that Death is going after them because they escaped a disaster they were meant to die in. Molly was always supposed to live, so she is not on the list. The only way out, he says, is to kill someone and take over the time they had left. The most dreaded scene is Olivia's, in the middle of the movie: a laser eye surgery machine malfunctions and damages her right eye, and she falls through a window before anyone can save her. Sam figures out that the deaths are following the order in which they would have fallen from the bridge, which puts Nathan next.
Nathan finds a loophole, or thinks he does
In a workplace argument, Nathan accidentally kills a coworker named Roy Carson, and the others conclude that Nathan has taken Roy's remaining lifespan. Sam works out that Dennis is next, and a machine fires a wrench into Dennis's face. That evening Sam and Molly get back together over dinner, until Peter shows up, hoping to take Molly's lifespan because he resents her for being the one who was meant to survive. Agent Block steps in and Peter shoots him, which gives Peter Block's lifespan. Peter still goes after Molly to remove any witnesses, and Sam saves her by impaling Peter on a meat spit. Sam then reasons that Peter's gun misfired because he had taken Block's lifespan.
The ending, explained
A fortnight on, Sam and Molly get on a plane to Paris and see a teacher leading six high school students off the plane, which tells Sam it is Volée Airlines Flight 180. Sam hears that one of the students was removed after panicking over a vision of the plane exploding. By the time Sam and Molly work out what is happening, nothing can be done, and the plane explodes with everyone aboard. At a memorial for Roy, Nathan learns that Roy had a brain aneurysm nobody knew about and could have died at any moment, so Nathan's plan to steal time never worked. As he works that out, landing gear from Flight 180 drops through the roof and flattens him. Every survivor of the bridge is dead.
