What actually happens
A premonition that comes with a logging truck
One year after the Flight 180 explosion, college student Kimberly Corman is driving to Daytona Beach in Florida with Shaina, Dano and Frankie, her friends, for spring break. While merging onto U.S. Route 23 she has a vision of a huge pile-up that starts with a logging truck. She stalls her car and gets out, which holds back eight other drivers: the state trooper Thomas Burke, the teacher Eugene Dix, Rory Peters, Kat Jennings, Nora Carpenter and her son Tim, the lottery winner Evan Lewis, and the pregnant delivery driver Isabella Hudson. While Burke is questioning her, the pile-up happens anyway. This is the scene people dread most, and it is at the start. A car carrier hits Kimberly's car, and Shaina, Dano and Frankie are killed. Burke pulls Kimberly clear just in time.

Death keeps a list, and the list has a psychiatric ward
The survivors go home, and Evan is impaled by a fire escape ladder when he runs from a stovetop fire in his apartment. Kimberly can tell Death is nearby, so she goes to see Clear Rivers, who has been hiding in a psychiatric ward after a falling brick got Alex Browning, which left her as the only Flight 180 survivor still alive. Clear explains that Death is picking people off in the order they would have died on Route 23. Death appears to be working backwards, though, because Kimberly's friends died before Evan, while in Kimberly's vision they died after him. Kimberly survived because Burke pulled her away, so Clear warns that Burke's rescue only puts Kimberly next on the list after him. She tells Kimberly to watch for omens and try to save herself.
A gun that refuses to take part
Kimberly sees omens of Tim's death but cannot stop a glass pane from crushing him. Clear leaves the ward and brings Kimberly and Burke to the mortician William Bludworth, who says only new life can beat Death. Burke decides that Isabella's baby is the loophole, so he holds her in custody on false charges to keep her safe, and he moves the other survivors into his apartment. Nora leaves anyway, and a chain reaction closes the elevator doors on her neck. Eugene then tries to shoot himself with Burke's gun, which misfires every time. Clear concludes that it is simply not his turn.
A farm with unlucky fencing
When they learn that Isabella has been taken to the hospital to give birth, the survivors drive there in Kat's SUV. On the way they work out that the deaths of the Flight 180 survivors accidentally saved them from earlier dangers before Route 23, and that Death is fixing this by reversing its list. A tire bursts, and the SUV ends up in a farm belonging to the Gibbons family. Kat is stuck in her seat while everyone else climbs out, and Eugene is taken to the hospital. A rescue worker then accidentally causes a PVC pipe to impale Kat. Her death sets off a chain reaction that sends barbed wire flying toward Rory, and he is dismembered.
The ending, explained
Kimberly has a vision in which Dr. Kalarjian seems to kill Isabella, so she, Clear and Burke hurry to the hospital. They see Isabella deliver a baby boy and decide the ordeal is finished. Then Kimberly has another vision, and it shows that Isabella was never meant to die on Route 23. An oxygen leak in Eugene's ward explodes, killing both Eugene and Clear. Kimberly realizes she has to die clinically and be brought back, since that would create the loophole. Kimberly sends an ambulance into a lake, Dr. Kalarjian revives her, and both she and Burke are saved. Later the two have a picnic with the Gibbons family and Kimberly's father. The Gibbons tell them Rory once saved their son Brian from a speeding news van. Kimberly and Burke realize what that means, and Brian is killed when a faulty propane grill explodes while he is cooking.
