What actually happens
Staged scares at a very real hospital
Two boys stream a live exploration of the abandoned Gonjiam Psychiatric Hospital, where the rumor is that the director, who loved ping-pong, murdered every patient and then vanished. They go to Room 402, which is the intensive care unit and no one has ever managed to open. A ping-pong ball is heard, the stream ends, and the viewers get a quick look at a ghostly face. The news of their disappearance prompts Ha-joon, who runs the YouTube channel Horror Times, to investigate. He recruits six people, Ah-yeon, Charlotte, Ji-hyun, Sung-hoon, Seung-wook and Je-yoon, and stays at a base camp to run the broadcast. As in earlier Horror Times videos, he has set up fake scares with Sung-hoon and Seung-wook. Inside, the group makes a few staged discoveries to raise the tension. Outside, the camp's propane burners switch on by themselves, and Ha-joon's equipment fails, cutting him off from the group.

A closet with a hole in it grabs back
Je-yoon and Ah-yeon try to open Room 402 while the others search the common room, where the closets have holes in them. When Ji-hyun puts her arm into one, something pulls and scratches it. Ji-hyun and Charlotte get scared and leave the asylum. Back at camp, Ha-joon looks at footage that shows all six participants together and becomes unsettled, because he can't tell who filmed it. He still persuades Seung-wook and Sung-hoon to keep going with the scripted stream by promising them more money. In the supply room, objects move on their own, and Seung-wook is knocked out cold. Sung-hoon gets dragged out of the room and is knocked out too.
The marked tree is a very consistent landmark
Outside the hospital, Ji-hyun and Charlotte keep coming back to the same marked tree, which tells them they are going in circles. Ji-hyun goes into a trance, and her eyes go black. Charlotte runs toward the camp and ends up back inside Room 402, where more scares follow. Then a naked man shows up and pulls her off into the dark. Sung-hoon wakes up and hurries over to Je-yoon and Ah-yeon, both of whom are still working on Room 402. He tells them, and the camera, that some of the earlier scares were staged, but that the paranormal activity is real now and they have to get Seung-wook out. Someone throws a ping-pong ball at them, and Charlotte screams from inside Room 402. Their infrared cameras pick up a presence as the door finally opens.
A drone shows Ha-joon a window with a view
The other three, Sung-hoon, Je-yoon and Ah-yeon, end up in a dark room full of water with no way out, and ghostly figures surround and possess them one at a time. Ha-joon sees his view count approaching a million and heads for the asylum. His drone camera spots someone struggling near a window on the top floor, so he goes in to look. Through a window he sees himself watching the earlier footage of the struggling man. This is the scene people dread most, and it comes near the end. Ha-joon freezes while the director's floating corpse creeps up behind him. He hesitantly turns the camera around to film himself, and the director's ghost screams and strangles him. Later, Seung-wook wakes up tied to a wheelchair and gets pulled into Room 402 while ghostly patients watch from their rooms.
The ending, explained
The livestream actually ended abruptly, right after Sung-hoon admitted that it was scripted. Viewers who know nothing about what happened inside the asylum mock it as a failed stream. It peaked at 503 viewers, far fewer than the nearly one million Ha-joon thought he saw. Ha-joon has been strangled by the director's ghost, Seung-wook has been dragged into Room 402, Charlotte has been pulled into the darkness, and Sung-hoon, Je-yoon and Ah-yeon have been possessed. Ji-hyun was last seen in a trance outside.
