What actually happens
A visitor center with a house in the basement
Five friends are retracing the route of Clover's missing sister, Melanie. They are Clover, her ex Max, Megan, and a couple, Nina and Abe. At a gas station where Melanie once sent a message, the attendant, Hill, tells them people tend to go missing around Glore Valley, an old mining town. Heavy rain drives the group into a visitor center in the valley. Abe finds a wall of missing-person posters, including one of Melanie, and Nina signs the guestbook. Downstairs they find another house underneath the building. A masked attacker then goes after them, and by the end of the night all five are dead.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
The hourglass keeps score
The friends wake up at the start of the same night. There is a new signature in the guestbook, their own missing-person posters are on the wall, and an hourglass in the main room shows that the night is repeating. A supernatural force possesses Megan, and the same force drags Clover into a run-down house across the road. An elderly resident there offers Clover a choice between living through the night and joining it, then gasses her and falls apart. Max tries to get Abe and Nina to help find Clover, but they run for the car instead. A huge creature in the woods turns them back, and the attacker catches them. Max finds Clover on his own. She is possessed, and she kills him before the masked attacker runs her over.
Tap water has side effects
The night starts again, and the group works out they are in a time loop that ends after thirteen deaths. This is the scene people dread most, and it comes in the middle: they lock themselves in the bathroom, drink the tap water, and every one of them explodes. Just before Clover dies, Hill appears and informs her that waiting out the night will not work. On the next loop, the group finds a recording in which Hill studies a man trapped in a room. Over thirteen nights, the man goes mad and turns into a wendigo. Hill contacts Clover by radio and says they can escape if one person dies for good. He tells her to come to the woods by herself, and hints that she may get to see Melanie. The group argues, Nina impales Abe with a pickaxe for trying to leave them, and Abe and Nina are murdered again. In the woods, a wendigo attacks Max and Megan, and Megan dies. Clover and Max then find Melanie, who has become a wendigo herself. Clover tells Max to finish Melanie off so they can find out more, and he cuts her throat. Then he drinks the water and explodes.
Hill has a therapy practice, apparently
The group learns that a mining disaster dropped Glore Valley underground, and that Hill was hired as a therapist for the survivors. On their thirteenth night, they wake up having forgotten the previous loops, and their bodies are beginning to turn into wendigos. Abe checks footage on his phone, which shows they have tried to find Hill on several nights and were slaughtered by creatures each time. Megan is missing, and they discover she survived a night and followed Hill into the tunnels. The group follows her trail and fights off wendigos along the way. Clover splits off and has to kill Melanie. She reaches a sanatorium, and there Megan is stuck with a wendigo while Hill tells her he is running experiments. The creatures they have been fighting are manifestations of Clover's depression and fears.
The ending, explained
Clover moves Hill's coffee mug under a pipe that drips water from the ceiling, and Hill explodes when he drinks from it. She frees Megan while the others finish off the masked attacker. The whole group runs through the tunnels and gets out as the hourglass empties and the sun rises, which ends the time loop. They drive away alive. Afterward, the monitors in Hill's office switch to a snowy cabin with a car arriving outside, and Hill can be heard whistling.
