What actually happens
A phone call that begins with a very personal question
The movie starts at a gas station in Mexico, where a young woman buys cigarettes. The cashier takes a phone call, turns to her and asks "Truth or dare?" in a demonic voice, and the woman uses lighter fluid to burn another woman alive. That is the scene people dread most, and it happens before the main story begins. College student Olivia Barron then lets her best friend Markie Cameron talk her into a spring break trip to Cancún. They bring Markie's boyfriend Lucas, whom Olivia likes, and friends Tyson, Brad, Penelope and Ronnie. Over drinks, Olivia runs into Carter, who leads the group to the abandoned Rosarito Mission to play truth or dare. Tyson outs Olivia's crush on Lucas, and Carter chooses truth and admits he was assigned to find a group to play. As they leave, he warns Olivia that the group has to obey the game's rules or die.

The game finds everyone at school
Back on campus, Olivia sees her classmates with demonic grins asking her "Truth or dare?" She picks truth and reveals that Markie cheats on Lucas all the time, which damages their relationship. Ronnie dies after failing to complete a dare. Markie breaks Olivia's hand, Brad is made to come out to his dad, and Tyson dies for failing to tell the truth. Online, the group finds an article about Giselle Hammond, the missing woman behind the murder in Mexico. Markie messages her with threats against her family to get a meeting. When it is Penelope's turn, she picks truth but is forced into a dare anyway.
Giselle brings a gun to a game
Giselle tells the group her friend Sam drunkenly wrecked the church, after which they played a version where nobody can pick truth twice in a row, and Carter was dared to recruit a new group, which put off their turns. She pulls a gun on Olivia and accidentally shoots Penelope. Lucas and Brad tackle her, and she is possessed and takes her own life. Lucas and Markie dig up a 1960s article about a massacre at the mission, in which Inez Reyes was the only survivor. Upstairs, a possessed vagrant attacks Olivia and dares her to sleep with Lucas. She tells Markie, who storms off. During the dare, Lucas is forced to admit that he loves Markie, which upsets Olivia. Afterward, Lucas and Olivia go to Tijuana to see Inez.
A spell that costs somebody a tongue
Inez no longer speaks, but she gets across that the priest running the convent regularly abused the girls and that one of them summoned a demon named Calux, who carried out the massacre. She gives them a spell that stops Calux, and whoever released him has to give up their tongue. Back home, Brad dies during his dare. Then Olivia is forced to tell Markie her biggest secret: the night Markie's father killed himself, Olivia had threatened to expose him for trying to sleep with her, and she told him Markie would be better off if he were dead. A detective questions Olivia and tells her that Carter's real name is Sam. That night, Olivia, Markie and Lucas kidnap Sam and take him back to Rosarito.
The ending, explained
At the mission, Olivia holds Sam at gunpoint and makes him perform Inez's spell. Calux possesses Lucas, who kills Sam and then himself, and the demon moves on to Markie. It tells Olivia that the game will not end until every player is dead. To delay their turn, Olivia films herself telling the whole story and ends it by asking the viewer, "Truth or dare?", which exposes the world to the game. By then Ronnie, Tyson, Penelope, Brad, Lucas, Sam and Giselle are all dead, and Markie is possessed.
