What actually happens
A surprise visit nobody asked for
Annie is an American musician and conspiracy theorist who livestreams while she drives, making up songs on the spot out of whatever her chat types. Fed up with pandemic rules and with the homelessness she sees in Los Angeles, she flies to London to surprise Stretch, an old friend and ex-bandmate who has become a food delivery driver. Stretch's girlfriend Gemma disagrees with Annie about politics and social issues from the first minute. Annie then rides along on one of Stretch's deliveries and manages to upset the restaurant owners with her anti-mask opinions.

A delivery job that was not hers
Back at the flat, Annie refuses to take off her hat, and Gemma attacks her. That night Annie hears Gemma telling Stretch to throw her out, so she takes Stretch's car and phone and accepts a delivery job that was meant for him. At a closed restaurant, the owner pays her to take an elderly woman, Angela, to an address the owner has written down. Angela soils herself, which forces a stop at a diner, where Annie notices a tattoo on Angela's stomach. A woman walks in looking for Angela and attacks Annie, who runs out with Angela.
A family reunion with a steering wheel in it
Stretch finds Annie through her livestream and gets into the car, and after an argument they pull over and Angela disappears. Stretch finds her perched at the top of a tree in the woods, falls while climbing toward her, and then watches her drift down slowly. The woman from the diner shows up with a shotgun, and they escape with Angela, then find her mouth is stapled shut. When they flag down a passing car, it runs over Stretch, and the driver is the same woman, who says she is Angela's mother and that Angela is sixteen. Annie crashes the woman's car, and the two of them wedge the woman's arm into the steering wheel, and it snaps by accident. This is the scene people brace for: Angela turns up, tears her mother's head from her body, and goes after Annie and Stretch.
An abandoned amusement park, then a house with a guest list
Angela chases them into an abandoned amusement park and kills Stretch. Annie escapes by car, but Angela makes her crash, then uses her telekinetic powers to shove the car into a lake. Annie traps Angela inside and swims to shore, without noticing Angela rising out of the lake behind her. Annie finds a remote house that she recognizes as the address she was supposed to deliver Angela to. Inside she finds occult symbols and a group of cultists who slash their own throats while she watches. Angela goes for Annie, who stabs her with a knife one of the cultists left behind.
The ending, explained
After Annie kills Angela, a large, slug-like creature crawls out of Angela's mouth and chases her. Annie finds Stretch's car, takes her keyboard from the back seat and finishes it off by hitting it with the keyboard until it stops. She then collapses into the car, drives away and picks her livestream back up, improvising a rap about everything that just happened. Stretch and Angela's mother do not survive, and neither do the cult members or Angela.
