What actually happens
Check-in at the Venus motel
In 2021 a woman named Claire stops in Venus, Oregon, and checks into the local motel, where the clerk is Shelly Barnes. That night someone knocks and asks if Tamara is home. Claire says no, and the three masked locals, Pin-Up Girl (Shelly), Dollface (Jasmine) and Scarecrow (Gregory Rotter), take her to an underground lair and kill her. In the present, after Shelly's death, Maya hides in a church, where Gregory turns up to mourn Shelly and startles her. Sheriff Rotter detains her, and she steals his patrol car because she doesn't trust him, then crashes it. Gregory and Jasmine kidnap her. Flashbacks explain that Gregory is the sheriff's son. Gregory took the blame for a murder Shelly committed, and the sheriff covered for both of them as long as they left Venus townspeople alone.

A woodchipper and a very bad tattoo
Gregory and Jasmine drive Maya to a sawmill and put Shelly's body through a woodchipper. They keep Maya in their underground hideout, where Gregory gives her the smiley face tattoo and makes her wear Shelly's old mask. More flashbacks show that Gregory and Shelly met Jasmine when she was a teenager passing through town. They trailed Jasmine and her boyfriend to the motel with murder in mind, only to find that Jasmine had handled the boyfriend already, so she teamed up with them. Meanwhile Maya's sister Debbie and her husband Howard drive in from Portland, after learning Maya never reached the city. A waitress named Annie at Carol's Diner tips them off about the sheriff, and they tail him along with their bodyguard, Marcus. Sheriff Rotter kills his own deputy, Tommy Walters, after the deputy figures out what the sheriff has been helping with.
A second motel, and the same question
Gregory ties Maya up and takes her in the truck with Jasmine to find someone new. They pick out a couple at a gas station and follow them to the motel. They coach Maya to play Pin-Up Girl at the door with the usual question about Tamara. The couple won't open the door, so Gregory smashes it in and ties both of them to chairs. This is the scene people dread most, and it happens in the middle of the movie: he tells Maya to choose one of them to die, she refuses, and he kills the man himself. When Jasmine looks away, Maya grabs Jasmine's own machete, stabs her with it and then slams her head into the floor, which finishes it. Gregory then makes Maya kill the woman with Jasmine's machete. After that he puts her back in the truck, where Sheriff Rotter tells him that Debbie and Howard are tailing them.
A trailer that was never going to stay upright
Debbie and Howard shelter in an abandoned trailer while Marcus goes to look around, and Sheriff Rotter kills him at the sawmill. Gregory shows up at the trailer, taunts the couple and flips it over with his truck. He breaks in, kills Howard with his axe, drags Debbie outside and kills her while Maya watches from the truck, tied up. Gregory puts Jasmine's body in the sheriff's car, then unties Maya and leaves her a knife and the truck. Maya puts Debbie's body in the truck and heads out of town. Then she realizes the murders will keep going, turns around and drives back to the underground hideout.
The ending, explained
Maya goes into the hideout with a shotgun that had been taken from Ryan and left in the truck. She shoots the sheriff dead, then walks into Gregory's shrine, where Gregory takes his mask off. The two of them talk about how each has had everything they cared about taken by the other. Maya tells him the shotgun is empty, and they embrace while she pretends to be interested in him. Then she stabs him with the knife he left her, which was the plan all along, and finishes him with his axe. Maya leaves the lair carrying his mask. Debbie, Howard, Marcus, Jasmine, Shelly and Tommy are all dead by this point.
