What actually happens
The airport has a friendly stranger and a plot
Nadja, a German widow who seems to be suffering from leukemia, is flying to New York with her young son Elias to see a doctor. The medicine she takes at the airport makes her visibly unwell. Elias befriends a man named Farid while they wait to board. Once everyone has settled in for the night, several men, one of them the co-pilot Bastian, kill the sky marshals and wreck the black box so that the plane disappears from radar. Their leader, Berg, tells the passengers to sit tight while a ransom gets paid. Elias hides, and Nadja follows him. A hijacker named Eightball shoots Nadja and leaves her for dead. The hijackers then make Farid record a statement that makes it look like a suicide attack, because they expect the plane to be shot down.

A broken-down car and a husband who does not come back
Nadja drifts back to awareness and remembers the day her husband Nikolai died. Their car broke down and he went to a nearby farmhouse for help. When he did not come back, Nadja went to look for him and was bitten by his killer, a vampire who died at sunrise. Nadja, now a vampire herself, returned to the farmhouse to find answers. An elderly vampire, whose son she had killed, tried to stop her to keep the evil from spreading. She beat him to death instead and escaped with vials of a vampire suppressant.
The cargo hold has two dogs and no good options
Nadja goes down to the cargo hold and takes out the contacts and false teeth that hide her mutated eyes and fangs. Two dogs are caged there. This is the scene people dread most. It comes in the middle of the movie: Nadja takes the smaller dog out of its cage, it squeals, and the scene cuts away. Next you see her at a puddle of blood, and later the hijackers find the dog dead, assume the other dog did it, and shoot that one too. A hijacker also catches her feeding, and she kills the man and drinks his blood, and that completes her transformation. She finds Farid, and with a student named Mohammed helping, she takes back control of the plane just as the hijackers get ready to jump out with parachutes. Mohammed puts them back on course. When Berg tries to return to the cockpit, Nadja bites him, then stabs him before he can transform.
Eightball takes a blood transfusion with him
The hijackers tell Eightball to kill Nadja. He knocks her down with UV light, takes her blood and is about to stake her when Elias accidentally depressurizes the plane by firing Berg's gun. Mohammed suffocates, and the hijackers retake the cockpit and bring the plane down far enough to restore the pressure. Eightball retreats to the cargo hold and injects Nadja's blood into himself. She tries to stop him and fails, and he becomes a vampire. He kills the other hijackers, apart from Bastian. The remaining passengers pick up whatever weapons they can find. Nadja convinces them that she is on their side, and together they lock Eightball in the hold.
The plane has no fuel and the sun has not come up
Bastian tells Nadja the plane cannot reach New York and has to land soon. A dying passenger lets Eightball out in the hope of being turned, and Eightball kills him and starts turning others instead. Nadja decides to blow up the plane with the hijackers' explosives, but Elias goes for the detonator himself and ends up surrounded. She gets Farid to promise he will watch over Elias, then kills and feeds on Bastian. Eightball goes after her and drinks her dry. While Eightball closes in on Elias, Farid turns the plane toward the morning sun, and Eightball burns and falls to his death. Afterward Elias gives Nadja his own blood to revive her, and she runs from him, afraid of how much she wants it. The plane comes down on an RAF airfield in Scotland, where soldiers who ignore Elias and Farid's warnings board the plane and are massacred. Elias returns and sees his mother feeding on a soldier.
The ending, explained
Elias sees that his mother is too far gone, and he sets off the detonator that he has been hiding inside his teddy bear. The blast seems to kill her and the rest of the vampires. Earlier, Eightball drained Nadja and then burned and fell to his death when Farid steered the plane into the sunrise. Elias's blood was what brought Nadja back, and afterward she fled from him because her thirst for it put him in danger. Berg, Bastian and the other hijackers are dead, Mohammed died when the plane depressurized, and Farid gets out of custody and gives Elias a hug.
