A burning woman and a very bad evening for a patrol car
A man named James runs out of a farmhouse and into the woods. A screaming woman chases after him and is shot, and Vincent and Simon pour gasoline on her and light her on fire. Deputy Daniel Carter finds James crawling along a road and takes him to the hospital, which has mostly been abandoned since a fire and is running with a skeleton crew. His estranged wife Allison works there as a nurse. Dr. Powell, nurse Beverly, intern Kim, a pregnant patient named Maggie, her grandfather Ben and a patient named Cliff are also inside. Daniel walks in on Beverly, in a trance and with her face flayed, attacking Cliff. She comes toward Daniel and he shoots her dead. Then he has a seizure and sees a strange vision.

Robed visitors and a nurse who got up again
Mitchell, a state trooper, comes to the hospital to pick up James after seeing the farmhouse. When Daniel steps outside to call in Beverly's death, a robed cultist tackles and stabs him, and he gets back inside as the building is surrounded. Beverly's body has turned into a tentacled creature. Vincent and Simon arrive in the lobby with guns and demand James. James takes Maggie hostage and stabs Powell, who falls and dies. The Beverly-Creature takes Mitchell, and Vincent and Simon kill it and set Mitchell's body on fire. While Allison goes to the basement for supplies to deliver Maggie's baby, Powell, who is no longer dead, captures her. Daniel and Vincent find files that show Powell led the cult. Powell then calls Daniel from the morgue and mentions the vision Daniel had.
A basement full of the wrong kind of patients
Daniel, Vincent and Simon take James downstairs to question him, and James says Powell can transform people. Meanwhile Allison wakes up strapped to an operating table, and Powell tells her he found a way to defy death after losing his daughter Sarah. He shows Allison that something is now growing in her belly. In the basement, the men find a hidden area full of deformed bodies that have come back to life, and one of the creatures kills James. Upstairs, Maggie goes into labor and Kim hesitates over a C-section. Maggie stands up and slits Ben's throat, and it turns out she is carrying Powell's child. She leaves to join him. Daniel finds Allison pregnant, with a tentacled creature coming out of her body, and he attacks her remains with an axe. He is then pulled into a morgue room where a glowing triangle hangs on the wall, and Powell offers him his child back if he agrees to die first.
A glowing triangle that nobody should have recited at
Maggie stabs Daniel, who is hurt but alive. Powell, skinless and partly mutated, stands in front of the triangle while Maggie kneels. He recites an incantation, and Maggie's torso explodes as she gives birth to the Sarah-Creature. That is the scene people dread most, and it comes near the end. Vincent and Simon arrive and fight it, and the creature overcomes Vincent. Vincent covers it in isopropyl alcohol and Simon lights both of them with a flare. Powell tells Daniel that Allison can be his again if he quits resisting. Daniel refuses and tackles him.
The ending, explained
Daniel tackles Powell, and the two of them fall into the void. The Sarah-Creature goes after Simon, who gets away and finds himself teleported back to the hospital, where he is reunited with Kim. Vincent is burned along with the creature. The last shot of Daniel and Allison has them hand in hand in another world, under a black pyramid. Powell's fate is left unknown.
