What actually happens
A clown does the laundry
In a morgue, Art the Clown comes back to life and brutally kills the coroner examining his body. He then goes to a laundromat to wash his blood-soaked suit, where he meets the Little Pale Girl, a mysterious entity dressed in a similar clown outfit. A year later, teenager Sienna Shaw is preparing a Valkyrie costume that her recently deceased father designed. Her little brother Jonathan has become fixated on Art after finding drawings of him in their father's sketchbook. During the night Sienna dreams about a commercial for the Clown Cafe, where Art attacks the patrons and she pulls a sword out of a cereal box. She wakes up to find her dresser on fire, and the sword resting on it is untouched.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
Trick or treat, with a second visit
The same day, Jonathan spots Art and the Little Pale Girl in the school grounds with a dead opossum, and the principal blames Jonathan when she finds the carcass. Meanwhile Sienna visits a costume shop, where Art antagonizes her. When she leaves, Art kills the clerk. The scene to brace for comes next, early in the story: Art turns up at Allie's door in the guise of a trick-or-treater. Later he breaks in, brutally maims her, and then kills her mother. It is brutal, and it all happens before Sienna leaves for the party.
A sketchbook full of useful clues
Jonathan shows Sienna their father's sketchbook, which is full of drawings of Art and newspaper clippings about Emily Crane, a ten-year-old who was murdered. Jonathan is convinced their father knew how to stop Art and gave Sienna the sword for that reason. Their mother, Barbara, scolds Jonathan for his fixation on Art. Sienna leaves for a Halloween party, and Jonathan runs away after Barbara slaps him. Art shoots and kills Barbara. When Jonathan comes back home, Art kidnaps him and takes Sienna's sword.
A haunted house named Terrifier
Brooke spikes Sienna's drink at the party, then has a panic attack when she spots the Little Pale Girl among the guests. Her boyfriend Jeff offers to drive the girls home, and a call comes in for Sienna from the Little Pale Girl, who imitates Jonathan's voice and sends them to an abandoned carnival. Sienna goes looking for Jonathan alone. Meanwhile Art kills Jeff in a gruesome way, then follows Brooke into a haunted house called Terrifier, where he kills her too. Then he beats Sienna unconscious.
The ending, explained
Sienna wakes up to find Art attacking Jonathan. She fights Art, Jonathan shoots him, and Sienna passes out. Art kidnaps Jonathan again, stabs Sienna with that same sword and shoves her through a portal to Hell, where she sees the Clown Cafe again. Sienna dies, but the sword resurrects her. She decapitates Art while he is chewing on Jonathan's legs, and the Little Pale Girl walks off with Art's head while Sienna and Jonathan hug. In a mental institution, a woman named Victoria writes "Vicky + Art" on the wall in her own blood, and then gives birth to Art's living head, her eyes glowing.
