What actually happens
Vicodin from a friend with a barbell plate
Six days after Rose Cotter's death, Joel, who is now cursed, tries to pass the Smile Entity along by killing one violent criminal in front of another. The plan goes wrong when Joel accidentally kills the witness during a shootout, and Lewis Fregoli, a drug dealer standing nearby, inherits the curse. Joel tries to run when more criminals show up and is killed by a pick-up truck. Meanwhile, New York pop star Skye Riley is preparing a comeback tour after a very public fight with addiction and a car crash that killed her boyfriend, the actor Paul Hudson. Her mother and manager, Elizabeth, watches her constantly, so Skye slips away to get Vicodin from Lewis, her friend from high school, because she hurt her back at rehearsal. This is the scene people dread most, and it comes early. At his apartment Lewis starts screaming, the Entity takes him over, and he gets up smiling and kills himself with a barbell plate while Skye watches. That passes the curse to her, and she leaves without telling anyone, afraid of being found with a body and a room full of drugs.

A nurse proposes stopping her heart as a cure
Skye starts seeing smiling versions of Paul, a disfigured Lewis and her assistant Joshua, and her mental state gets worse fast. She makes up with her estranged friend Gemma. Then a text from an unknown number claims to know that she visited Lewis's apartment and warns her she is in danger. Skye meets the sender at a bar. He is Morris, an ER nurse who has been tracking the Entity since it killed his brother. He explains the curse and suggests a fix: stop her heart and then restart it, on the theory that the Entity is a parasite that will die without a host. Skye refuses and leaves when bargoers recognize her. Back at her apartment the Entity comes for her in the shape of her backup dancers and throws her around the rooms, and a giant arm forces its way down her throat until she blacks out.
The retreat, the mirror and a very long drive with the wrong Gemma
A flashback shows that Skye caused Paul's death by crashing the car on purpose during an argument while she was on drugs. She wakes up at a retreat and argues with Elizabeth about the tour. Elizabeth smiles, smashes a mirror and stabs herself with a shard, and when Skye turns to run she sees that she is the one holding it. Hospital workers and Joshua see her covered in blood. Skye escapes, reunites with Gemma and steals a car to go see Morris. Then a call from the real Gemma tells her that the Gemma who has been with her all week is the Entity. She takes back control of the car and gets to Morris at an abandoned Pizza Hut, and there he intends to put her in the walk-in freezer, which should protect her brain while he stops her heart. While he is out, the Entity appears as Skye's old self from the car wreck. She wrestles it off, then gives herself the injection that was supposed to stop her heart, and the Entity tells her she still isn't in control.
Opening night at Herald Square Garden
Skye suddenly finds herself on stage for the first show of her tour at Herald Square Garden. In the audience she sees Elizabeth, who is alive, and works out that the whole day was an illusion. The Entity shows up looking like Skye as she is now. It tears open its own stomach, and its real body comes out: a giant, skinless creature roughly shaped like a person, with several smiling mouths spread over it. Skye screams and freezes into a trance, and the Entity takes her over.
The ending, explained
Once the Entity possesses Skye, she smiles and drives her own microphone into her eye in front of thousands of horrified spectators. Skye dies, and the curse wins. Joel and Lewis are dead before this point, and Paul died in the car crash that Skye caused. The Gemma who spent the week with her was the Entity the whole time.
