What actually happens
A rabbit with a grudge
Sarah is a divorced fertility doctor, and she and her daughter Mia find a rabbit at their house on Mia's seventh birthday. Mia loves it. That night Mia's dad Peter arrives with his new partner Denise and her three-year-old, Toby, and Mia screams after Toby whacks her on the head. Later Sarah tries to send the rabbit over the garden wall, and it bites her. She realizes Mia saw her do it. From the next morning Mia keeps going unresponsive, wears a rabbit mask she made herself and won't take it off, and hides in a playground tube at school. She also says she misses her grandmother Joan, whom she has never met and whom Sarah has not spoken to in years.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
A family tree with an extra branch
Mia wants photos for a family tree, and in the pile Sarah finds a childhood picture of herself and her younger sister. Mia says she is in the photo. Mia gets more erratic, Sarah starts hallucinating and having worse nightmares, and the rabbit bite on her hand gets infected. Mia keeps insisting on seeing Joan, so Sarah takes her to the home where Joan lives. Joan has dementia and calls Mia Alice, and Mia reacts strongly. Sarah pulls them apart and hurts Mia's hand, then hurts her own hand to match. Mia's teacher tells Sarah about disturbing drawings on the back of her homework, and Sarah finds the same drawings on all of Mia's artwork at home.
The shed that Sarah left out of every story
Sarah confronts Mia about the behavior, and Mia announces that she is Alice. Peter suggests a trip, so Sarah drives Mia to her childhood home and describes her sister Alice, who went missing at the age of seven. Mia keeps insisting she is Alice, and she accuses Sarah of bullying her. Sarah's hallucinations get worse, and she cuts Mia with scissors by accident. Then comes the scene to brace for, near the end, and it plays as Sarah's hallucinated memory. As a child, Sarah locked Alice in a shed. After Sarah let her out, Alice choked her, Sarah struck Alice with a steel trap, and Alice's screams ended when Sarah pushed her off a cliff.
An apology to the wrong daughter
Peter arrives to find the family photos smashed and Sarah drawing on the floor, barely aware of what she is doing. They search for Mia while Sarah hallucinates Alice drowning, and they find Mia in a bush. Sarah visits her mother Joan at the nursing home and admits that Alice is dead. She returns to her childhood home and apologizes to Mia, who she is now treating as Alice, for hurting her and for lying about how Alice ran off. Mia tells her she is a monster. By morning Sarah is searching for Mia again, and from the window she sees Mia and Alice walking toward the cliff, holding hands.
The ending, explained
Sarah finally tells the truth. She visits Joan and admits that Alice is dead, then goes back to her childhood home and apologizes to Mia, who she is treating as Alice, for hurting Alice and for the lie about her leaving. Mia answers by calling Sarah a monster. The next morning, Sarah looks for Mia and, from a window, sees Mia and Alice holding hands as they walk toward the cliff.
