What actually happens
A grieving couple adopts a girl from the local orphanage
Kate and John Coleman live in Hamden, Connecticut, and their marriage has been strained since their third child, Jessica, was stillborn. Kate is a recovering alcoholic and has taken the loss especially hard. They decide to adopt, and they choose Esther, a nine-year-old Russian girl from a local orphanage called St. Mariana's Home for Girls. Their six-year-old daughter Max, who is deaf, takes to Esther. Their twelve-year-old son Daniel does not.

The orphanage director visits and does not get to leave
Esther's behavior gets worse in small steps. She interrupts Kate and John in their bedroom, then shows more knowledge of adult matters than a nine-year-old should have. She kills an injured pigeon in front of Max and Daniel, and she breaks the ankle of Brenda, a classmate with a bullying streak. Then Sister Abigail, who runs the orphanage, visits the house to tell Kate and John that bad things keep happening near Esther, including the fire that wiped out her previous adoptive family. This is the scene people dread most, and it comes early. When Abigail goes to leave, Esther causes her car to crash, then kills her with a hammer. She makes Max help move the body to Daniel's treehouse. Daniel sees them, and Esther threatens him that night so he will keep quiet.
A bouquet cut from the baby's grave
Kate grows more certain that something is wrong with Esther, and John decides she is being paranoid. He asks Esther to be nice to Kate, so Esther rips the flowers off Jessica's grave and hands them over as a bouquet. Kate grabs her arm, and that night Esther breaks her own arm in a vice and blames Kate. The next day she releases the car's brake so it rolls into traffic with Max inside, and she tells everyone about the wine she found in the kitchen. Their therapist suggests rehab, and John threatens to leave and take the children. Kate finds out that Esther came from a mental hospital called the Saarne Institute, and that the orphanage has no record of her.
Daniel's treehouse and a very unhelpful hospital stay
Daniel learns about Sister Abigail from Max and goes to search the treehouse. Esther, who overheard them, lights it while he is still up there. He falls while escaping, and Esther tries to finish him until Max stops her. In the ICU, Esther tries to suffocate him and his heart stops before the doctors revive him. Kate attacks Esther and is sedated. That night Esther tries to seduce John, which finally convinces him that Kate was right, and he threatens to send her back. At the hospital, a doctor from the Saarne Institute tells Kate that Esther is Leena Klammer, a 33-year-old Estonian woman with a hormonal disorder that stunted her growth. She has posed as a child for most of her life, has killed at least seven people, and burned down the home of the Estonian family that adopted her.
The ending, explained
While Kate is talking to the doctor, Leena wipes off her makeup and pulls out her fake teeth. She destroys her room, and the ribbons were hiding scars from trying to break out of restraints. The power goes out and she disappears. John finds her hidden paintings with a blacklight, and she stabs him to death downstairs. Kate comes home, finds John, and searches for Max. Leena shoots Kate in the arm, then fires at Max in the greenhouse. Kate drops through the greenhouse roof and knocks her out. Kate and Max run as the police pull up, but Leena catches them by the frozen pond. Max tries to shoot her but hits the ice, and Kate and Leena go into the water. Leena goes back to being Esther and begs her Mommy not to let her die, but Kate says she is not her mother and kicks her in the face, breaking her neck. She sinks, and the police find Kate and Max.
