What actually happens
A gift for the new girlfriend
Dani Odello-Timmis, the wife of the psychiatrist Ted Timmis, is alone in the couple's new country house when Olin Boole, one of Ted's former patients, drops in to warn her that a stranger has gotten into the house. Dani is then brutally murdered, and Olin, believed to be the killer, is later found in a halfway house with his head smashed. A year later, Ted goes to the Cabinet of Curiosities shop in Cork run by Dani's twin Darcy, and hands her Olin's glass eye. Darcy is a blind clairvoyant who reads objects, and she uses the eye to see Dani's last moments. Several days later she turns up at the house where Ted lives with his new girlfriend Yana, having sent them a chest holding a life-sized wooden golem as a gift.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
The golem rearranges itself
Ted leaves for his night shift at the hospital. Yana is uncomfortable and tries to leave, but she cannot find her car keys. She also notices that the golem keeps changing position during the evening. When she examines it, she finds strange objects in five holes in its head: pictures of both sisters, hair clippings, a tooth and some blood in a vial. Darcy angrily demands that Yana put everything back, and she does before going to bed. Yana then sees Dani's apparition, which leads her to her car keys inside the golem's crate, and she flees the house in a panic.
Ted explains himself, badly
When Ted gets back, Darcy accuses him of being behind Dani's murder. She says she killed Olin, but when she read his glass eye she learned he was innocent. She believes Ted hired Ivan, a brutish orderly from the psychiatric hospital, to kill Dani. That way Ted could be with Yana and keep the house instead of losing it in a divorce. Olin had overheard Ted and Ivan discussing the plan and went to warn Dani, which did not work. Ted offers to bring in the investigator to prove his innocence, and he heads to his office, leaving his phone behind with Darcy. He calls the phone, and when Darcy goes to get it she falls through a trapdoor Ted had removed and lands on the stone floor, badly hurt.
Ivan meets the golem
This is the scene people dread, and it is near the end. On Ted's orders Ivan goes to the house to make sure Darcy is finished, and the wooden golem stalks and attacks him, apparently animated by Darcy. Darcy dies of her injuries shortly afterward. Ivan survives and Ted has him committed to the psychiatric hospital, where he rambles about the golem, which has lost its power. Ted burns the golem. Then, to tie off loose ends, he lets a violent cannibal out, who kills and eats Ivan. Yana ends their relationship, disturbed that Ted is willing to live in a haunted house where two people have died.
The ending, explained
Ted goes back to the country house and lives there alone. On the doorstep he finds a small box from Darcy's shop with a call bell inside, one she had shown him earlier and described as haunted by a bellhop who kills whoever rings it. To prove his rationalist beliefs, Ted rings the bell. Nothing seems to happen and he relaxes, unaware that the ghost of the bellhop is standing right behind him. Darcy and Dani are dead, Olin is dead, Ivan is dead, and Yana has left.
