What actually happens
A cheap room with some decorative sacrifice
After her mother dies, Ambar, who is undocumented, leaves Mexico for Cleveland, Ohio. She works an under-the-table job and lives on her savings. She rents a run-down boarding house room from Red, who makes her pay the first month in advance. After she moves in, strange things happen. One night a man bangs his head against a door, and she later learns he is Becker, Red's brother, who is mentally ill. Some rooms hold odd artifacts, including depictions of ritual sacrifice, and ghostly figures start appearing around the house. Wanting a better-paid job, she hands a co-worker the rest of her savings for a fake ID that says she is a U.S. citizen. The co-worker takes the money and disappears, and when Ambar presses her boss for the co-worker's address, he fires her.

A refund that comes with conditions
Scared by visions, Ambar leaves the house. She is broke and her distant uncle Beto can't help, so she calls Red. They meet at a diner, and Red says he will refund her deposit but only if she comes back to the house, since he has no cash on him. When they get there, the money isn't in her room. Becker appears and makes her drink some wine, and she locks her door. Later two Romanian women who have moved in knock and talk with her about dreams of a stone box, and Petra sings until Ambar falls asleep. She wakes to find Red and Becker restraining the women to take them to the basement. She attacks Red, and Becker throws her against a wall. Beto arrives and Becker assaults him. Red then explains that his archaeologist father brought back a stone box from Mexico in 1963, and that his parents murdered women for it. Those women are the ghosts Ambar has seen. Each woman sacrificed to the box improves Becker's health, so Becker insists on staying.
A box with a moth in it
Becker hauls Ambar down to the basement, and there she sees the other woman's headless body. He straps her onto a stone slab facing an altar with the box on it, opens the box and leaves, shutting the door. Beto bursts in and unties her. As they try to get out, Ambar slips into a dream in which her mother is dying, while a large moth-like creature slowly rises from the box and holds her head in its hands. In the dream her mother begs her to stay and starts to strangle her, but Ambar fights back and suffocates her. This makes the creature, an Aztec goddess, pull back into the box. Ambar snaps out of the dream and finds herself alone beside the altar.
A weapon from the study and a table in the basement
Ambar hears Becker and Red getting Petra ready for sacrifice. Becker notices his wounds aren't healing and says something is wrong. Ambar picks up a weapon called a macuahuitl in the study and heads upstairs, where she hurts Red and Becker attacks her. Petra tries to help, and Becker throws her over the balcony, killing her. Becker chokes Ambar, but she slices his neck open with one of its obsidian blades and finishes by smashing his head. Then she hears Red, who is hurt but still alive, moving about next door. This is the scene people dread most. Near the end, she takes Red downstairs, ties him to the stone table and watches the moth-like creature bite off his head.
The ending, explained
On her way through the house, Ambar passes Red, who is now a ghost, like the women sacrificed before him. As she starts to leave, her ankle, which Becker broke earlier, suddenly heals, because she gave Red to the monster. She stops and holds out her hand so a moth can land on it. Then she turns back toward the basement door, and her veins are dark and throbbing, just like Becker's. Becker, Red and Petra are dead, and Ambar is the one walking back toward the basement.
