What actually happens
A methane check skipped for a dance
Valentine Bluffs is a Canadian mining town, and twenty years before the movie starts two supervisors at Hanniger Mine left their posts to go to the Valentine's Day dance. They forgot to check the methane levels in the tunnels, an explosion collapsed a tunnel, and every miner died except Harry Warden. Warden survived by resorting to cannibalism and was driven insane by it. The next year he killed the two supervisors who had skipped their shift, cut out their hearts and put them in Valentine's candy boxes. He left a note warning the town never to hold the dance again, or more killings would follow. Harry was then locked up in an asylum, and the town obeyed the note for two decades.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
The mayor opens a box of chocolates
Mayor Hanniger decides the dance can come back, and the town's young people get excited. That includes Sarah Mercer and Axel Palmer, along with Gretchen, Dave, Hollis, Patty, Sylvia, Howard, Mike, John, Tommy, Harriet plus T.J., the mayor's son, who is stuck in a love triangle with Sarah and Axel. The mayor and Chief Jake Newby then receive a box of chocolates with a human heart in it, along with a note saying the killings will start if the dance happens. That evening a miner in full gear kills a woman named Mabel in a laundromat and takes her heart. Newby tells the town she had a heart attack. He calls the asylum, which has no record of Harry Warden. The dance is cancelled, so the young people decide to hold a party at the mine instead. A bartender tries to warn them off and is killed while setting up a dummy miner.
A party in the mine, with a pot of hot dogs
The party is the scene to brace for, and it happens in the middle of the movie. The miner kills Dave in the kitchen by pushing his face into a pot of boiling hot dogs, and his heart is later found in the pot. Sylvia is killed in a shower. When the others realize Dave and Sylvia are dead they call the authorities, but several partygoers have already gone into the mines for fun. T.J. and Axel go in after them. Mike and Harriet are killed by a large drill. Hollis, who went looking for them, takes a nail gun to the head, and Howard runs away in horror. Patty, T.J., Axel and Sarah try to climb out on a ladder, and Howard's body, tied to a rope, drops down from above.
The killer turns out to be on the guest list
As the survivors look for a way out, Axel seems to drown, and the miner finishes off Patty with a pickaxe. The miner then goes after T.J. and Sarah, and they fight. The miner turns out to be Axel, who faked his death. His father was one of the two supervisors Harry Warden killed, and Axel saw it happen as a child, which scarred him badly. T.J. throws a rock at Axel and the tunnel caves in on him. Chief Newby and the police turn up to get T.J. and Sarah out, and Newby informs them that Harry Warden had been dead for five years. So Warden was never behind any of this. Then a rescuer shouts that Axel is still alive, and T.J. and Sarah run back to the scene.
The ending, explained
T.J. and Sarah run back to the collapsed tunnel and watch Axel pull himself out of the rubble by amputating the arm that was pinned. He disappears further into the mine, shouting that he and Harry Warden will come back and murder everybody in town. He also mumbles that Sarah is his bloody valentine. Axel is loose in the mine and still making threats.
