What actually happens
A hospital with two boys and one blood problem
At a hospital in Greece, ten-year-old Michael Morbius greets Lucien, the boy who becomes his surrogate brother, and renames him Milo. They both have a blood illness, and both want to be normal. The hospital director, Dr. Emil Nicholas, who is also their adoptive father, sees how gifted Michael is and sends him to medical school in New York, while Nicholas stays at the hospital to care for Milo. The two boys are both patients, and Michael is the one the director notices.

Gene splicing on a mercenary ship
Twenty-five years later Michael turns down a Nobel Prize for his work on artificial blood. His colleague Martine Bancroft finds out that he has secretly collected dozens of vampire bats from Costa Rica, because he wants to splice their genes into his own and cure his illness. Milo funds a private mercenary vessel in international waters to hold the equipment. The scene people dread most is the one that follows, early in the movie. The cure works, but Michael becomes a vampire, and when the crew attacks him out of fear, he drains them of blood. When his hunger passes, he is horrified, erases the CCTV footage of the experiment, contacts the authorities and jumps overboard.
FBI agents and one brother with a cure
Back in New York, Michael finds he now has superhuman strength and speed as well as echolocation, and his bats treat him as one of their own. He tries to control the hunger with artificial blood, but it satisfies him less and less. FBI agents Simon Stroud and Al Rodriguez investigate the crew's deaths and work out that Michael was involved. Milo discovers that Michael is cured and gets angry when Michael won't cure him too. When Michael visits Martine in the hospital, he finds a nurse who has been drained of blood. He thinks he did it, tries to run and is arrested by Stroud. In prison Milo visits, and Michael realizes that Milo has taken the cure for himself and is the one who killed the nurse. Michael breaks out to face him, but Milo is not sorry and wants Michael to enjoy his powers. Michael cannot hurt him and leaves.
An antibody and an army of bats
Michael meets Martine, gets a new lab and works out an antibody against vampirism, partly to stop Milo and partly to use on himself. The agents come across video of one of Milo's attacks and give it to the press, convinced that Michael's condition is spreading. Nicholas recognizes Milo and begs him to stop, but Milo is angry that Nicholas always seemed to prefer Michael, and he gives Nicholas a fatal wound. Michael is too late to save him, and Milo fatally wounds Martine as well. She dies in Michael's arms, and he has to drink her blood. Michael calls a swarm of bats to hold Milo down and injects him with the antibody. Milo dies.
The ending, explained
After Milo dies, Michael flies away with the bats, grieving for the people he lost and accepting that he is a vampire. He does not know that Martine has come back to life, because she swallowed a drop of his blood while he was feeding on her. Dr. Nicholas and Milo are dead, and Martine is alive again. Michael is a fugitive. In two scenes in the middle of the credits, Adrian Toomes is pulled into Michael's universe from his own. He decides another hero is to blame, finds Michael, and proposes that they form a team.
The post-credits ruling
Yes, there are two scenes in the middle of the credits. A man from another universe shows up and suggests teaming up with Michael.
β Spoiled safelyDog status: fine, no dog dies.
