What actually happens
A tailgater with a pipe and a trench coat
Trish Jenner and her brother Darry are driving through the Florida countryside on their way home for spring break, and they pass the time talking about Kenny and Darla, a prom couple who vanished in 1978 and became a local legend. An old truck tailgates them hard and then pulls ahead. Later they see the same truck parked beside an abandoned church. The driver, in a big trench coat, is dropping bundles wrapped in bloody sheets into a pipe sticking out of the ground. He notices he has an audience, chases them, and forces their car off the road. Trish would like to leave, which is the reasonable position.
Once the driver is gone, Darry talks Trish into going back to the church. They hear strange noises coming from the pipe, so Darry climbs in while Trish holds his feet. He slips out of her grip and lands in the basement. This is the scene people remember, so here is exactly what it is: a dying man with stitches down his stomach, and a crowd of corpses stitched onto the walls and the ceiling, Kenny and Darla from the legend among them. It happens early, before the monster has been seen. Darry and Trish then head to a diner.
The driver has opinions about their laundry
The siblings go to a diner and take a cryptic call from a woman warning them of danger. She mentions a cat lady and the song "Jeepers Creepers", and says one of them will meet a terrible fate while it plays. Darry calls the police, and a waitress tells them the truck driver broke into their car and sniffed their laundry. Two officers escort them back to the church, but the church has been set on fire, and on the way back the driver attacks and kills both officers. Trish and Darry pull up at the house of an older woman with lots of cats and ask to use her phone. The driver gets in, kills her, and shows them what he really looks like. Trish runs him over several times, and a giant wing rips through his coat. They drive away.
The police station was a bad idea
At a police station they meet Jezelle Gay Hartman, the psychic who phoned them. She explains that their pursuer is the Creeper, an ancient creature that wakes once every 23 springs and spends 23 days feeding on human body parts, which it absorbs into itself. It picks victims by their fear. Sgt. Tubbs, the officer in charge, decides she is delusional. That night the Creeper breaks into the holding cells and kills several prisoners to heal its wounds. The police cannot stop it. It traps the siblings in an interrogation room upstairs, sniffs them both, and throws Trish aside. Trish offers to go instead of Darry, but the Creeper has made up its mind and flies out of a window with him.
The ending, explained
The next day Trish's parents pick her up, and Jezelle goes home. In an abandoned factory, Darry's body is shown mutilated: his back is cut open and his eyes are gone. As the song "Jeepers Creepers" plays, the Creeper looks out through the holes where his eyes used to be. Trish is the one who survives.
