What actually happens
The road moved while nobody was looking
Becky, who is pregnant, and her brother Cal hear a boy named Tobin crying for help from a field of tall grass. His mother Natalie is with him. The siblings go in and are separated almost immediately, and the distances inside the field behave strangely. They decide to leave and cannot find the road. Becky runs into Ross, Tobin's father. He says to stay close, and she loses sight of him almost at once. Cal meets Tobin holding a dead crow. Tobin buries it, says the grass does not move dead things, and says a rock told him Becky will die soon. Tobin takes Cal to the rock and says to put a hand on it. Then Cal hears Becky scream, and a ghost attacks him.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
A very long detour
Travis, the father of Becky's baby, enters the field to search for the siblings. He finds Tobin, who acts as if they know each other, and Tobin leads him to Becky's body, repeating that the field does not move dead things. Meanwhile Tobin's parents arrive at a nearby church with the family dog, Freddy. When Travis shouts Tobin's name from the grass, Freddy runs in, and the others chase after him and get separated. The siblings go back into the grass too, and Tobin announces that Freddy has died. Travis, Becky, Cal and Tobin find each other, and Travis informs the siblings that they have been gone for two months.
Ross knows a way out and it involves the rock
The group sees a building in the distance. Becky gets a phone call from someone telling her to stop making the same mistake, then the grass seems to enter her body and she passes out. Ross revives her, reunites with Tobin, and says he will lead everyone out. He takes them to the rock, where Natalie warns them not to touch it and says Ross is dangerous. Travis attacks Ross, who dislocates Travis's arm and crushes Natalie's head. Ross claims the rock told him everything, including the way out, but he has decided to stay.
Travis picks a bad roof to apologize on
Becky, Cal, Travis and Tobin make it to the building. Travis had asked Becky to abort the baby and she refused, so she and Cal were driving to meet a family who might adopt it. Travis says he is sorry and wants to be part of the child's life. On the roof, Travis and Cal find a hole that Freddy used to get out to the road. Cal lets Travis's hand go as he slips, and Travis falls off the roof. Tobin runs into the grass when Ross follows them up, and Becky will not abandon Travis, so she turns back while Cal runs off. Ross strangles Cal. The group is in a time loop, with Cal hunted forever by a possessed Ross. Travis survives and starts looking for Becky.
Labor in a thunderstorm, next to the rock
This is the scene people dread, and it comes late in the movie: Becky's labor at the rock, followed by what happens to her baby. Ross attacks Becky and she escapes, and a thunderstorm begins. Grass creatures carry her to the rock, which has marks that relate to her pregnancy. She phones her past self and begs her to stop Cal from hurting Travis. As she screams through contractions, the ground under the rock opens and roots turn into humanoid figures reaching for her. She passes out and wakes to Cal holding her baby, passes out again, and wakes to see Cal feeding her the baby and telling her it is only grass and seeds. Becky realizes this "Cal" is Ross. Tobin finds Travis and tells him that Ross killed the baby and Cal and will keep doing it over and over. Ross badly wounds Travis and tries to make Tobin touch the rock. Becky attacks Ross and then dies of her wounds. Travis strangles Ross with a bundle of grass, touches the rock to see its visions, leads Tobin to an exit, and says he is staying behind.
The ending, explained
Tobin comes out across the road just as Becky and Cal are about to walk into the grass. He shows them Becky's necklace, which Travis gave him, and convinces them to stay out. That closes the loop, and Becky decides to keep the baby while they drive home. Back in the field, Travis listens to them leave and then dies. In the version of events inside the grass, Natalie, Cal and Becky are dead and Ross has been strangled by Travis.
