What actually happens
The murder capital next door to the nice town
The movie begins in 1994 at the Shadyside Mall, where Ryan Torres stabs his friend Heather, who works at the bookstore, and goes on to kill six other employees before Sheriff Nick Goode shoots him. The media says this is normal for Shadyside, while neighboring Sunnyvale is rich and has a reputation as one of the safest towns around. Many Shadyside teenagers blame a curse from Sarah Fier, a witch executed in 1666. Deena Johnson doesn't believe it, and she has recently broken up with her girlfriend Sam, who is closeted and has moved to Sunnyvale. Her brother Josh spends his time on local history, while her friends Simon and Kate deal drugs to save up for leaving. At a vigil in Sunnyvale, a fight breaks out between the two schools. On the way home, Sam's boyfriend Peter tailgates the Shadyside bus, Deena nearly throws a cooler at his car, a nosebleed makes her drop it, and Peter crashes. Sam lives through the crash and has a vision of Sarah Fier before she is sent to a hospital.

The old killers come back for a blood sample
The next night, Deena and her friends think Peter and his friends are stalking them. Then Skull Mask kills Peter, and two more people at the hospital, and is revealed to be Ryan, now undead. Sam and Deena can't convince the police, and Simon is attacked by Ruby Lane, a Shadyside killer from 1965. The group works out that the crash upset Sarah Fier's grave and that Sam's blood landed on her bones, which brought several of the town's killers back. Josh concludes the curse explains Shadyside's long history of murders. They try to rebury Fier's remains properly, and the Camp Nightwing Killer shows up to stop them. They realize the killers only want Sam and are drawn to her blood, so they set a trap at the school and burn them. The killers get back up.
A plan involving a drowning and a pharmacy
Sam agrees to be sacrificed, over Deena's protests, but the group stops her once they learn that C. Berman, who lived through the 1978 Camp Nightwing massacre, claimed to have seen the witch. They call Berman and get no answer. Sam finds out that Berman died once and was resuscitated, so the group plans to kill Sam and revive her with drugs from a supermarket pharmacy. This is the scene to brace for, near the end. While Deena prepares, Kate, Simon and Josh fight off the Shadyside killers in the store. Skull Mask kills Kate with a bread slicer, and the Camp Nightwing Killer hits Simon in the head with an axe. Deena finally drowns Sam, the killers disappear, and Deena brings her back with EpiPens and CPR.
A phone call from the survivor
Afterward the police blame Simon and Kate for the killings because they were known drug dealers. Sam and Deena get back together and come out as a couple in public. Sam is at her own house when Deena gets a call from C. Berman, who warns her that nobody escapes the witch. Sam is now possessed and attacks Deena, but Deena subdues her and ties her up, and promises to work out how to undo it.
The ending, explained
Deena drowns Sam to make the killers disappear, then brings her back with EpiPens and CPR. Kate and Simon are dead. The police blame Kate and Simon for the killings because they sold drugs. Sam and Deena get back together and come out as a couple. That night Deena gets a call from C. Berman, who says there is no escaping the witch. Sam, possessed, attacks Deena, who ties her up and promises to find a way to free her. The curse is not settled, and Sam is still possessed.
