What actually happens
Group therapy with a very attentive doll
Andy Barclay has spent four years torturing the severed head of Chucky. Meanwhile Nica Pierce, a paraplegic survivor, sits in a mental institution after Chucky framed her for killing her family, and she now believes she did it herself and that Chucky was only in her head. Her psychiatrist, Dr. Foley, who sexually abuses her, has her moved to a medium-security facility. There she meets the other patients: Angela, Claire, Madeleine, and Malcolm, who has dissociative identity disorder. Foley introduces a new therapy technique in group, using a Good Guy doll. Madeleine, who was locked up for smothering her infant son, immediately treats the doll as her baby.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
Tiffany drops off a second doll
Tiffany Valentine, the legal guardian of Nica's niece Alice, visits and tells Nica that Alice has died. She also leaves a second Good Guy doll, and Chucky's spirit is inside it. That night Chucky finds that Nica has slit her wrists, and in the morning her wrists are stitched and Angela is dead, so Nica realizes Chucky was real all along. Chucky kills Claire next, and Malcolm starts calling himself Charles, which makes Nica suspect Chucky has moved into him. Andy learns of the murders and works out that Chucky has transferred his soul into several vessels at once. Foley hypnotizes Nica and tries to assault her, but Chucky knocks him out and tries to coax Nica into killing him. Madeleine smothers her doll with a pillow, which forces her to face her real child's death, and the orderlies bury the doll. Andy gets himself committed by assaulting a guard. A nurse, Carlos, delivers a third doll to Foley, and Madeleine's own doll comes to see her, and she lets it kill her.
Stomping with a new set of legs
Foley tries to assault Nica again, and a Chucky doll knocks him out. Nica realizes that both his doll and Tiffany's doll are alive, and they tell her that the first Chucky discovered a voodoo spell that divides his soul between several hosts, and Alice was one of them. Carlos wanders in, and the three dolls kill him. This is the scene people dread most, and it comes late: one of the Chucky dolls puts a share of his soul into Nica, which gives her body the ability to walk again, and she brutally stomps on Foley's head until he is dead. Malcolm then admits that he is not really Charles, who is just another of his split personalities, and one of the dolls kills him.
A gun in the wrong doll
The third doll opens Andy's cell and goes for him. Andy tears the doll's chest open and takes out a hidden gun, which shows that he sent the doll to the institution to smuggle a weapon in. He kills it, but he cannot get out before Nica-Chucky locks him in his cell and leaves him there. Nica-Chucky escapes and reunites with Tiffany, and the pair drive away with a Tiffany doll that is also alive and carries a piece of her soul. In a scene after the credits, Kyle, who used to be Andy's foster sister, goes to his house, sent to keep torturing the severed Chucky head.
